È necessaria la consultazione del medico per risolvere il problema della co-somministrazione di medicinali.
Consumatore:Con PEMEtrexed insieme con ibuprofene può aumentare gli effetti di PEMEtrexed. Rivolgersi al proprio medico se si verificano febbre, brividi, dolori muscolari, sintomi influenzali, ulcere della bocca o macchie bianche, e diarrea grave. PEMEtrexed può abbassare le cellule del sangue che aiutano a combattere le infezioni. Questo può rendere più facile per voi a sanguinare da un infortunio o di ammalarsi di essere in giro gli altri che sono malati. Per essere sicuri che le cellule del sangue non troppo bassa, il tuo sangue sarà bisogno di essere testati regolarmente. Se il vostro medico non prescrivere questi farmaci, potrebbe essere necessario un aggiustamento del dosaggio o speciali di test di utilizzare in modo sicuro entrambi i farmaci. Professionista:REGOLARE l'INTERVALLO di somministrazione: la somministrazione concomitante con farmaci anti-infiammatori non steroidei (Fans) possono aumentare le concentrazioni plasmatiche di pemetrexed. Il meccanismo non è stato descritto, ma possono essere correlati ai FANS inibizione di le prostaglandine renali. L'uso di Fans è stato associato con nefropatia si manifesta come aumenti della creatinina sierica e BUN, necrosi tubulare, glomerulitis, renale, necrosi papillare, acuta, nefrite interstiziale, sindrome nefrosica e insufficienza renale. Dal pemetrexed è principalmente eliminato immodificato per escrezione renale, la somministrazione concomitante con Fans può causare ritardi e/o riduzione della clearance. Quotidiano ibuprofen dosaggio di 400 mg quattro volte al giorno ha dimostrato di ridurre il pemetrexed la liquidazione di un 20% circa in pazienti con funzione renale normale, considerando che l'aspirina 325 mg ogni 6 ore, non ha fatto. L'effetto di dosi più elevate di ibuprofene o aspirina è sconosciuto.
GESTIONE: alla dose di 400 mg di Ibuprofene e aspirina 325 mg quattro volte al giorno, o meno, può essere utilizzato con pemetrexed in pazienti con funzione renale normale. Tuttavia, si consiglia cautela nei pazienti con lieve o moderata insufficienza renale (clearance della creatinina da 45 a 79 mL/min). Questi pazienti devono evitare l'assunzione di Fans con breve eliminazione di mezza vita (ad esempio, diclofenac, etodolac, fenoprofen, flurbiprofene, ibuprofene, indometacina, ketoprofene, ketorolac, meclofenamate, acido mefenamico, sulindac, tolmetin, basse dosi di salicilati) 2 giorni prima a 2 giorni dopo il pemetrexed amministrazione. Se la somministrazione concomitante è necessario, i pazienti devono essere strettamente monitorati per la tossicità, soprattutto mielosoppressione, nefrotossicità, e tossicità gastrointestinale. In assenza di dati riguardanti l'uso con i Fans, con più di metà vita, ritenuta FANS di dosaggio per almeno 5 giorni prima a 2 giorni dopo il pemetrexed amministrazione è raccomandato.
